My First Visit
1. Make an appointment, use the online scheduling tool on the home screen 24-7, or you can call the office during our regular business hours (9am through 5pm, 7 days per week) at (239)-330-1382.
2. I need to be seen sooner than the indicated on-line availability, what do I do? Call the office at (239)-330-1382 during regular business hours and we will make every effort to fit you in. If you have a true emergency, then always call 911.
3. I have made my appointment, what's next? Please download our New Patient Forms, review and fill out. Please do NOT return these forms through e-mail, but rather bring them to your first appointment, or drop off at the office prior to your first appointment, if you have access to a FAX you can send to (239)-228-6952. If you prefer, the office can also email or fax you the new patient forms, just let us know.

4. What should I bring to my first appointment?
- completed patient forms
- all medications you are currently taking, preferably in their bottles, with labeled prescriber and pharmacy information, please note, the actual medication bottles are ALWAYS better than a written list, you might be surprised at how many times we have identified mislabeled medications or the pharmacy putting a medication in a bottle not matching the label
- any supplements or other over the counter agents you may be taking, also preferably in their bottles
- if you have been keeping a record/log of any health parameters, such as your blood pressure, weight or blood sugar, then please bring these
- any documents, records from previous physicians, X-Rays, labs etc., that you would like for us to review and scan into your electronic chart
- a written list of any specific questions or complaints you might have, , writing down your questions for your doctor will significantly help in getting the most out of your visit
